RESUMO

Family and Community Medicine is the most offered and chosen specialty in the MIR (Spanish medical residency examination), however, every year its attractiveness is questioned due to not all offered positions being filled and a certain number of resident doctors deciding not to continue in this specialty once started. In this context, some of the proposals to address the problem focus on increasing the supply when the facts show that the challenge lies in addressing the demand by making the specialty and its professional scope more attractive. The problem and its determinants are analyzed in this context by focusing on four elements that may be influencing it: the vocational aspects of medical graduates who pursue specialization, the characteristics of the specialty program and the teaching units where training is carried out, the presence of family medicine in the university as a key element for knowledge and affinity to this specialty from undergraduate studies, and finally, the situation of primary care as the space where training is materialized and the priority setting for the professional practice of future specialists.

BACKGROUND: In GP training, identifying early predictors of poor summative examination performance can be challenging. We aimed to establish whether external clinical teaching visit (ECTV) performance, measured using a validated instrument (GP Registrar Competency Assessment Grid, GPR-CAG) is predictive of Royal Australian College of General Practitioners (RACGP) Fellowship examination performance. METHODS: A retrospective cohort study including GP registrars in New South Wales/Australian Capital Territory with ECTV data recorded during their first training term (GPT1), between 2014 and 2018, who attempted at least one Fellowship examination. Independent variables of interest included the four GPR-CAG factors assessed in GPT1 ('patient-centredness/caring', 'formulating hypotheses/management plans', 'professional responsibilities', 'physical examination skills'). Outcomes of interest included individual scores of the three summative examinations (Applied Knowledge Test (AKT); Key Feature Problem (KFP); and the Objective Structured Clinical Examination (OSCE)) and overall Pass/Fail status. Univariable and multivariable regression analyses were performed. RESULTS: Univariably, there were statistically significant associations (p < 0.01) between all four GPR-CAG factors and all four summative examination outcomes, except for 'formulating hypotheses/management plans' and OSCE score (p = 0.07). On multivariable analysis, each factor was significantly associated (p < 0.05) with at least one exam outcome, and 'physical examination skills' was significantly associated (p < 0.05) with all four exam outcomes. DISCUSSION: ECTV performance, via GPR-CAG scores, is predictive of RACGP Fellowship exam performance. The univariable findings highlight the pragmatic utility of ECTVs in flagging registrars who are at-risk of poor exam performance, facilitating early intervention. The multivariable associations of GPR-CAG scores and examination performance suggest that these scores provide predictive ability beyond that of other known predictors.

Introduction: The opinion of students is of utmost importance to identify areas of improvement in undergraduate studies. Medical schools would use this information to plan actions to ensure that the students achieve the necessary medical knowledge. The aim of this study was to analyse the opinion of medical students about their learning process and to analyse the influence of their experience according to their year of medical degree. Methods: A questionnaire including 21 items, divided into four sections (motivation, theory lectures, hospital internships, and research) and two overall questions, was distributed among eligible 246 students. Each item was scored from 1 (strongly disagree) to 5 (strongly agree). The opinions of intermediate-year students of medical degree (3rd and 4th) were compared to late-year students (5th and 6th). Results: A total of 148 students answered the questionnaire (60.2% response rate). The mean scores for overall student motivation and teaching quality were 6.15 and 7.10, respectively. The student-teacher interaction and new learning technological tools were considered important for student motivation. The only differences found between the two groups of students were that late-year students wished to become part of a medical team and to learn writing scientific papers more than the intermediate-year students. Conclusions: This questionnaire revealed that the year of career had little influence on the medical students' opinion on their learning process during their undergraduate studies. Late-year students rated highest on being more interested in being part of a medical team and their knowledge on writing scientific articles. The use of new technologies and the student-teacher interaction is key to motivate students.

Objetivo: Evaluar la incidencia y factores asociados al pensamiento suicida en una muestra de médicos residentes de dos instituciones. Materiales y métodos: Se realizó un estudio de tipo descriptivo, observacional, prospectivo y transversal, entre los meses de septiembre a octubre del 2022, para estimar el pensamiento suicida en los médicos residentes de dos hospitales con la escala de Plutchik, además de buscar factores asociados. Se aplicó la estadística descriptiva con medidas de tendencia central y de dispersión, frecuencias relativas y absolutas; las pruebas ji al cuadrado de Pearson y de bondad de ajuste, así como la prueba de Kruskal-Wallis se emplearon para examinar las diferencias entre especialidades, y la prueba post-hoc de Tukey para evaluar la especialidad diferente. Resultados: Se respondió un total de 225 encuestas, de las cuales se eliminaron 20 por inadecuado diligenciamiento, y quedaron 205. El promedio de edad fue de 28,66 años (DS ± 2,360) y el 71,2 % correspondió al sexo femenino. En cuanto a las especialidades, se encontró a pediatría con el 28,8 % y a anestesiología con el 20,5 %. Se evidenció asociación significativa entre especialidades, con un valor de p = 0,0000, y grado académico de p = 0,003 (p ≤ 0,05). Según la especialidad, se encontraron diferencias en cuanto al pensamiento suicida; la prueba de Kruskal-Wallis mostró un valor de p = 0,000 y la prueba post-hoc de Tukey reveló que la especialidad de ginecología fue la diferente. Conclusiones: De acuerdo con los resultados de la muestra, alrededor de una cuarta parte de los médicos residentes manifiesta pensamiento suicida. La prevalencia en dicha muestra no presenta diferencia significativa con respecto a la incidencia a nivel latinoamericano. Se encontró una asociación entre ideas suicidas, especialidades médicas y grado académico. En cuanto a comparación entre las especialidades, ginecología fue la que mostró mayor ideación suicida. Este trabajo presenta algunas limitaciones, por ejemplo, existe una gran heterogeneidad de grupos, no se empleó una técnica de selección probabilística y las pruebas estadísticas empleadas fueron no paramétricas.


Objective: To evaluate the incidence and factors associated with suicidal ideation in a sample of resident physicians from two institutions. Materials and methods: A descriptive, observational, prospective and cross-sectional study was carried out to estimate the suicidal ideation and associated factors with the Plutchik Suicide Risk Scale among resident physicians from two hospitals between September and October 2022. Descriptive statistics were used with measures of central tendency and dispersion, as well as relative and absolute frequencies. In addition, Pearson's chi-square goodness of fit test and Kruskal-Wallis H test were used to examine the differences between specialties, and Tukey's Honest Significant Difference test to determine which specialty was different. Results: A total of 225 surveys were answered, out of which 20 were eliminated due to inadequate completion, leaving 205 complete surveys for analysis. The average age was 28.66 years (SD ± 2.360) and 71.2 % were females. Concerning the specialties, pediatrics was found in 28.8 % of the respondents and anesthesiology in 20.5 %. A significant association between specialties with a value of p = 0.0000 and academic degrees with p = 0.003 (p ≤ 0.05) was evidenced. Differences regarding suicidal ideation were found by specialty: Kruskal-Wallis H test showed a value of p = 0.000 and Tukey's Honest Significant Difference test revealed that the specialty of gynecology was the different one. Conclusions: According to the results of the study sample, approximately one fourth of the resident physicians had suicidal ideation. Its prevalence in this sample showed no significant difference with respect to its incidence in Latin America. An association between suicidal ideation, medical specialties and academic degree was found. As for the comparison between specialties, gynecology was the one with the highest suicidal ideation rate. This work had some limitations; for example, the groups were very heterogeneous, a probabilistic selection technique was not used, and the statistical tests were nonparametric.

A partir de 2024, egresados de facultades de medicina que deseen hacer estudios de postgrado en los EE.UU o el Canadá, deberán graduarse en escuelas de medicina con programas educativos de calidad avalados por agencias reconocidas capaces de otorgar una acreditación internacional. La World Federation for Medical Education (WFME) es una de estas agencias. La WFME aceptó la nueva política de acreditación del Educational Committee for Foreign Medical Education (ECFMG) por la que médicos que postulen para la certificación del ECFMG del 2024 en adelante, tendrán que haberse graduado en un centro universitario de medicina acreditado por una agencia de aseguramiento de calidad que se encuentre reconocida por la WFME. El COMAEM (Consejo Mexicano para la Acreditación de la Educación Médica) está avalado por la WFME y otros organismos internacionales que aseguran la calidad de la educación superior. La acreditación que concede el COMAEM es un reconocimiento que el programa de medicina cumple con los criterios, indicadores y parámetros de calidad establecidos por este organismo. A partir de 2024, los egresados de un programa acreditado podrán postular para la certificación del ECFMG a través del examen de licencia médica de los Estados Unidos o USMLE (United States Medical Licensing Examination) y así poder hacer una residencia de especialización o trabajar en EE. UU. En el Perú, solo la Facultad de Medicina Alberto Hurtado de la Universidad Peruana Cayetano Heredia ha completado el proceso de acreditación internacional a través de COMAEM y ha recibido dicha acreditación.


As of 2024, medical school graduates who wish to pursue graduate studies in the U.S. or in Canada, they must have graduated from medical schools with quality educational programs endorsed by recognized agencies, capable of granting international accreditation. The World Federation for Medical Education (WFME) is one of these agencies. The WFME accepted the new accreditation policy of the Educational Committee for Foreign Medical Education (ECFMG) whereby physicians applying for ECFMG certification from 2024 onwards, must have graduated from a university medical center accredited by a quality assurance agency that is recognized by the WFME. The COMAEM (Mexican Council for the Accreditation of Medical Education) is endorsed by the WFME and other international organizations that ensure the quality of higher education. The accreditation granted by COMAEM is a recognition that the medical program meets the criteria, indicators and quality parameters established by this organization. Starting in 2024, graduates of an accredited program will be able to apply for ECFMG certification through the United States Medical Licensing Examination (USMLE) and thus be able to do a specialty residency or work in the U.S. In Peru, only the Alberto Hurtado School of Medicine of the Cayetano Heredia Peruvian University has completed the international accreditation process through COMAEM and has received such accreditation.

Introdução: O enfermeiro possui um papel fundamental na prevenção de Infecções Sexualmente Transmissíveis incuráveis; porém, para que sua atuação seja efetiva, faz-se necessária uma educação de qualidade com infraestrutura adequada. Objetivo: Mapear estudos no âmbito mundial que abordem o ensino das Infecções Sexualmente Transmissíveis Incuráveis para estudantes de graduação em enfermagem. Metodologia: Protocolo de revisão do escopo de acordo com a metodologia do Joanna Briggs Institute. A estratégia de busca será aplicada nas bases de dados Medical Literature Analysis and Retrieval System Online, Cumulative Index to Nursing and Allied Health Literature, Embase, Web of Science e Literatura Latino-Americana e do Caribe em Ciências da Saúde; assim como bases de dados de literatura cinzenta (e.g., Research Gate, OpenGrey). Serão considerados estudos que abordem o ensino das Infecções Sexualmente Transmissíveis Incuráveis voltados para estudantes de graduação em enfermagem no mundo. Serão considerados estudos de acesso público disponibilizados de 1981 até 2020. Dois revisores independentes farão a triagem dos artigos de acordo com os critérios de inclusão pré-definidos, extrairão e farão a análise de dados. A apresentação dos dados será por meio de figura e quadros conforme o Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews. Resultados: Os resultados provenientes deste estudo possibilitarão o conhecimento amplo sobre a educação dos estudantes de enfermagem relacionada com as infecções sexualmente transmissíveis incuráveis, assim como com as metodologias utilizadas e efetividade das mesmas. Conclusão: Este estudo pode contribuir para melhoria de metodologias, infraestrutura e currículos de ensino sobre infecções sexualmente transmissíveis incuráveis para estudantes de enfermagem.

La estrategia de búsqueda se aplicará a las bases de datos Medical Literature Analysis and Retrieval System Online, Cumulative Index to Nursing and Allied Health Literature, Embase, Web of Science y Literatura Latinoamericana y del Caribe en Ciencias de la Salud, así como bases de datos de literatura gris (p. ej., Research Gate, OpenGrey). Se considerán estudios que aborden la enseñanza de las infecciones de transmisión sexual incurables dirigida a estudiantes de graduación en enfermería de todo el mundo. Así como, los estudios de acceso público disponibles desde 1981 hasta 2020. Dos revisores independientes examinarán los artículos, de acuerdo con los criterios de elegibilidad predefinidos, extraerán y analizarán los datos. Introduction: Nurses have a fundamental role in the prevention of incurable sexually-transmitted infections; however, for their performance to be effective, quality education with adequate infrastructure is required. Objective: To map out the worldwide studies that address the teaching of incurable sexually-transmitted infections to undergraduate nursing students. Methodology: Scoping review protocol according to the Joanna Briggs Institute methodology. The search strategy will be applied to the Medical Literature Analysis and Retrieval System Online, Cumulative Index to Nursing and Allied Health Literature, Embase, Web of Science and Latin American and Caribbean Literature in Health Sciences databases, as well as to the gray literature databases (e.g., Research Gate, OpenGrey). This document will address those studies addressing the teaching of incurable sexually-transmitted infections aimed at undergraduate nursing students around the world; it will also consider the public access studies from 1981 to 2020 available. Two independent reviewers will screen the articles according to pre-defined inclusion criteria; they will also extract and analyze the data. The presentation of data will be through figures and tables according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews. Results: The results from this study will grant broad knowledge about the teaching of incurable sexually-transmitted infections to nursing students, as well as the methodologies used and their effectiveness. Conclusion: This study can contribute to the improvement of methodologies, infrastructure, and teaching curricula of incurable sexually-transmitted infections to nursing students.

BACKGROUND: Graduate entry nursing programmes provide students with an accelerated pathway to becoming a registered nurse. Motivations for study, together with commonly shared characteristics of students enrolling in such programmes is becoming well documented, however, their experiences of studying for a professional qualification in this manner is less understood. As a means of maintaining the relevance of these fast-tracked programmes in the future, an understanding of graduate entry nursing students' experiences of academic teaching and clinical placements is imperative. OBJECTIVE: To explore the academic and clinical experiences of students enrolled in the first year of graduate entry nursing programmes in New Zealand and Australia. METHODS: A qualitative case study approach was taken. Here we report the experiences of nine students enrolled in their first year of a two-year graduate entry nursing programme during 2020. Semi-structured interviews were used for data collection and analysed using Braun and Clarke's thematic analysis. FINDINGS: Three overarching themes were developed-affirmation, reflections on expectations and clinical experiences. CONCLUSION: This study highlights the experiences of first year graduate entry nursing students, with many experiencing affirmation that their altruistic career visions came to fruition. The findings indicate that these graduate-entry nursing students interviewed for this study tended to be flexible and adaptable in their approach to study as a means of meeting the challenges of the programme, all of which are key characteristics for a registered nurse; with personal growth and the development of the self, providing preparation for their second year of study.

Objetivo: refletir sobre oportunidades para a internacionalização da Enfermagem em países lusófonos proporcionadas por uma rede internacional de conhecimentos em Enfermagem. Método: método reflexivo que conduz à percepção modificada de uma dada situação levando a novas ideias, revelando temas de análise e propostas de possíveis soluções através de um plano de ação. A reflexão concentrou-se em diálogos interdisciplinares, inovação conceitual-metodológica e fortalecimento da liderança em Enfermagem. Resultados: a reflexão focalizou: (a) o diálogo além do contexto cultural e científico lusófono para a Rede ser reconhecida como parceiro intelectual substancial; (b) a incorporação de abordagens, referenciais e desenhos de pesquisa multidisciplinares; e (c) a mobilização de conhecimento sobre causas sociais para o fortalecimento da liderança da Enfermagem global nas questões de justiça social e equidade em saúde. Considerações finais: a Enfermagem lusófona possui expertise para inovar com estratégias para reforçar a internacionalização.


Objective: reflect on opportunities for the internationalization of nursing in Lusophone countries provided by an international network of nursing knowledge. Method: a reflexive method leading to the modified perception of a given situation leading to new ideas, revealing themes of analysis and proposals for possible solutions through an action plan. This reflection focused on interdisciplinary dialogues, methodological-conceptual innovation, and strengthening of leadership in nursing. Results: the reflection focused on: 1) the dialogue beyond the Lusophone cultural and scientific context for the network to be recognized as a substantial intellectual partner; 2) the incorporation of multidisciplinary approaches, references, and research designs; and 3) the mobilization of knowledge about social causes to strengthen global nursing leadership in issues of social justice and health equity. Final considerations: Lusophone nursing has expertise to innovate with strategies to strengthen internationalization.


Objetivo: reflexionar sobre las oportunidades de internacionalización de la Enfermería en los países de lengua portuguesa proporcionadas por una red internacional de conocimiento en Enfermería. Método: método reflexivo que conduce a una percepción modificada de una situación que lleva a nuevas ideas, revelando temas de análisis y propuestas de posibles soluciones por medio de un plan de acción. La reflexión se centró en los diálogos interdisciplinarios, la innovación conceptual-metodológica y el fortalecimiento del liderazgo en enfermería. Resultados: la reflexión tuvo como foco: (a) el diálogo más allá del contexto cultural y científico de la lengua portuguesa para que la Red sea reconocida como un socio intelectual sustancial; (b) la incorporación de enfoques, referencias y diseños de investigación multidisciplinarios; y (c) la movilización de conocimiento sobre las causas sociales para fortalecer el liderazgo mundial de enfermería en temas de justicia social y equidad en salud. Consideraciones finales: La Enfermería em países de lengua portuguesa tiene el expertise para innovar con estrategias para reforzar la internacionalización.

Introducción: la ecografía es una herramienta fundamental en la asistencia al paciente crítico. La incorporación de la ecografía clínica al trabajo de los intensivistas y emergencistas debe ser validada a través del desempeño; por tanto, se hace necesario el desarrollo de las competencias profesionales en este ámbito. Objetivo: exponer los elementos teóricos derivados del desarrollo científico-técnico que fundamentan las competencias en ecografía clínica. Métodos: se realizó un análisis crítico reflexivo sobre el contenido de la bibliografía revisada. Se accedió a libros y artículos científicos de revistas indexadas en las bases de datos PubMed, Scopus, Medscape, SciELO, y Medline, fueron seleccionados 25 artículos científicos con suficiente calidad y actualidad, en su mayoría, de los últimos cinco años y pertinentes para el estudio. Se utilizaron las palabras claves: ecografía clínica, competencias, cuidados intensivos, ultrasonido de urgencia y educación médica. Desarrollo: la ausencia del enfoque por competencias en la enseñanza y aprendizaje de la ecografía clínica constituye una barrera para lograr la generalización de esta técnica en las unidades de cuidados intensivos y áreas de emergencias. La introducción de la ecografía clínica en cuidados intensivos debe tener un enfoque de educación basada en competencias en la educación de pregrado y posgrado con un programa estructurado, certificado y validado por los estándares internacionales. Conclusiones: es imprescindible que se universalice y haga sostenible la utilidad y necesidad de este enfoque, su realización y continuidad en el tiempo, a través de la educación permanente y continuada.


Introduction: ultrasound is a fundamental tool in critical patient care. The incorporation of clinical ultrasound into the work of intensivists and emergencies must be validated through performance; therefore, it is necessary to develop professional skills in this area. Objective: to present the theoretical elements derived from the scientific-technical development that support the competencies in clinical ultrasound. Methods: a reflective critical analysis was carried out on the content of the reviewed bibliography. Books and scientific articles from journals indexed in the PubMed, Scopus, Medscape, SciELO, and Medline databases were accessed, 25 scientific articles were selected with sufficient quality and topicality, mostly from the last five years and with relevance in the study. The keywords were used: clinical ultrasound, skills, intensive care, emergency ultrasound and medical education. Development: the absence of the competency-based approach in the teaching and learning of clinical ultrasound constitutes a barrier to achieve the generalization of this technique in intensive care units and emergency areas. The introduction of clinical ultrasound in intensive care must have a competency-based education approach in undergraduate and postgraduate education with a structured program, certified and validated by international standards. Conclusions: it is essential that the usefulness and necessity of this approach, its implementation and continuity over time, be universalized and made sustainable through permanent and continuous education.

Objetivo: refletir sobre o ensino remoto, utilizado na pandemia da covid-19, na pós-graduação stricto sensu no contexto da universidade. Método: estudo de reflexão, apoiado em levantamento bibliográfico que permitiu o reconhecimento do predomínio do enfoque técnico, analisado à luz de autores cujo referencial teórico articula a educação e a universidade às relações sociais. Resultados: a análise do ensino remoto demanda uma investigação acerca da sua relação com mudanças que vêm se processando na universidade no cenário neoliberal, valorizando projetos de educação e sociedade com foco mercadológico, influenciando a produção do conhecimento, a formação do pesquisador e do professor da educação superior. Conclusão: ultrapassar a dimensão técnica para a apropriação da dimensão ético-política presente no ensino remoto e nos seus possíveis desdobramentos pós-pandêmico é fundamental, pois o potencial do ato educativo, em termos do encontro entre professor e aluno e da apropriação do conhecimento, não é efetivamente alcançado no ensino remoto.


Objective: to reflect on remote teaching, used during the covid-19 pandemic, in stricto sensu graduate studies in the university context. Method: this is a reflection study, supported by a bibliographical survey that allowed the recognition of the predominance of the technical focus, analyzed in the light of authors whose theoretical framework articulates education and university with social relations. Results: the analysis of remote teaching demands an investigation into its relationship with changes that have been taking place at universities in the neoliberal scenario, valuing education and society projects with a marketing focus, influencing knowledge production, researcher and professor training. Conclusion: going beyond the technical dimension to appropriating the ethical-political dimension present in remote teaching and its possible post-pandemic developments is fundamental, as the educational act's potential, in terms of the meeting between professor and student and appropriation of knowledge, is not effectively achieved in remote teaching.


Objetivo: reflexionar sobre la enseñanza a distancia, utilizada durante la pandemia de covid-19, en los estudios de posgrado stricto sensu en el contexto universitario. Método: se trata de un estudio de reflexión, sustentado en un levantamiento bibliográfico que permitió reconocer el predominio del enfoque técnico, analizado a la luz de autores cuyo marco teórico articula la educación y la universidad con las relaciones sociales. Resultados: el análisis de la enseñanza a distancia exige una investigación sobre su relación con los cambios que se vienen produciendo en la universidad en el escenario neoliberal, valorar proyectos de educación y sociedad con enfoque de marketing, incidiendo en la producción de conocimiento, la formación de investigadores y docentes de educación superior. Conclusión: ir más allá de la dimensión técnica para apropiarse de la dimensión ético-política presente en la enseñanza a distancia y sus posibles desarrollos pospandemia es fundamental, ya que las potencialidades del acto educativo, en cuanto al encuentro entre docente y alumno y la apropiación del conocimiento, no se logra efectivamente en la enseñanza remota.

Fundamento: el sistema de evaluación es un componente importante del proceso enseñanza aprendizaje del Programa de Formación de las Especialidades Médicas. Objetivo: elaborar un sistema de acciones educativas para contribuir a la evaluación integral del residente de Oftalmología. Métodos: se realizó una investigación cualitativa en el Centro Oftalmológico de Villa Clara, en el período septiembre 2019-octubre 2021. Se emplearon métodos teóricos: análisis-síntesis, inductivo-deductivo, histórico-lógico y enfoque sistémico; empíricos: análisis documental, cuestionario y entrevista; matemáticos: frecuencias absolutas y relativas. Resultados: la totalidad de los docentes consideró importante fomentar la formación de valores, principios éticos y morales en los residentes. El 95 % no supieron argumentar las relaciones que se establecen entre el acápite I de aspectos generales de la tarjeta de evaluación y los demás componentes del proceso enseñanza aprendizaje; la mayoría confunden los métodos y medios con las formas de organización de la enseñanza; solo dos residentes tienen dominio de los aspectos que contempla el plan de estudio de la especialidad y todos tienen escasa percepción de la importancia del acápite I de la tarjeta de evaluación, por lo que se elaboró un sistema de acciones educativas teniendo en cuenta los objetivos, contenidos y métodos educativos para solucionar las carencias encontradas. Conclusiones: el diagnóstico realizado logró la identificación de insuficiencias y potencialidades en la evaluación integral del residente y su concreción en el acápite I de la tarjeta de evaluación; las acciones fueron valoradas satisfactoriamente por los especialistas consultados, quienes sugirieron aspectos que fueron adicionados al producto final.


Background: the evaluation system is an important component of the teaching-learning process of the Training Program for Medical Specialties. Objective: to develop a system of educational actions to contribute to the comprehensive evaluation of the Ophthalmology resident. Methods: a qualitative research was carried out at the Villa Clara Ophthalmological Center, from September 2019 to October 2021. Theoretical methods were used: analysis-synthesis, inductive-deductive, historical-logical and systemic approach; empirical ones: documentary analysis, questionnaire and interview; mathematics: absolute and relative values. Results: all the teachers considered it important to promote the formation of values, ethical and moral principles in the residents. 95% did not know how to argue the relationships established between section I of general aspects of the evaluation card and the other components of the teaching-learning process; most confuse the methods and teaching aids with the forms of organization of teaching; only two residents have mastery of the aspects contemplated in the study plan of the specialty and all have little perception of the importance of section I of the evaluation card, for which a system of educational actions was developed taking into account the objectives, content and educational methods to solve the deficiencies found. Conclusions: the diagnosis made achieved the identification of insufficiencies and potentialities in the comprehensive evaluation of the resident and its concretion in section I of the evaluation card; the actions were satisfactorily valued by the specialists consulted, who suggested aspects that were added to the final product.

Abstract BACKGROUND: The burnout syndrome can be avoided and/or have its signs and symptoms reduced by knowing the five associated factors that help identify the health and working conditions of the professors of graduate programs. OBJECTIVE: To analyze the factors associated with burnout among faculty members of graduate stricto sensu programs. DESIGN AND SETTING: A cross-sectional study was conducted among 585 faculty members of Graduate Programs in Language Teaching and Linguistics in Brazil. METHODS: Data were collected through an online questionnaire. The outcomes were the dimensions of burnout and its related factors identified through multiple templates of logistic regression. RESULTS: Faculty members with increased chances of emotional exhaustion and depersonalization mentioned the use of medications due to labor activities. The negative influence of pace and intensity of work, thoughts about quitting the program, and having to produce three or more scientific articles were associated with higher chances of emotional exhaustion, while having to achieve nine hours per week in undergraduate programs was related to reduced personal accomplishment. Having a conjugal relationship, satisfaction with health and work, post-doctoral degree, autonomy, and good interpersonal relationships with faculty members of the program reduced the chances of emotional exhaustion. Reduced chances of depersonalization occurred among those who were satisfied with work, had good interpersonal relationships with advisees and faculty members, and received productivity funding. CONCLUSION: Sociodemographic, health, and occupational factors related to the dimensions of burnout were identified.

The study utilized a cross-sectional design. Data was retrieved from the American Academy of Family Physicians fellowship training directory, including program names, areas of focus, program length, and program descriptions. The collected data was categorized and analyzed based on areas of focus and training duration. The program descriptions were also analyzed qualitatively using Nvivo12 software. A total of 532 programs were included in the study:295 programs (55.5%) were ACGME-certified, while 237 programs (44.5%) were not. The majority of programs, 468 (87.9%), were clinically oriented, while 64 programs (12.1%) were non-clinical. Among ACGME-certified programs, the largest number of programs were in sports medicine (121 programs), followed by geriatrics (52 programs). Among non-certified programs, the largest number of programs was in obstetrics (66 programs), accounting for 27.8% of all non-certified programs. Qualitative research found that fellowship programs were diverse, reflecting the societal demands of healthcare service. Moreover, a significant emphasis was placed on empowering teaching and research abilities.The family medicine fellowship programs in the United States cover a wide range of disciplines and meet both the professional interests of doctors and the needs of patients. As China continues to implement its tiered medical system, it can learn from the experience of the United States and develop general practice subspecialty training programs, thereby improving the service capacity of general practitioners and improve the quality of healthcare.

RESUMO Este artigo parte da compreensão de interdisciplinaridade como modo de operar face às visões fragmentadas presentes nos processos de produção e de socialização do conhecimento. Objetiva compartilhar reflexões que problematizam a interdisciplinaridade a partir da experiência de formação acadêmica na pós-graduação de um grupo de pesquisa cujos integrantes expressam diversidade de formação e inserção profissional e se debruçam sobre relações entre políticas públicas, saúde e necessidades das pessoas. As reflexões foram elaboradas com base em questões emergentes nos encontros sistemáticos do grupo, que foi tomado como estratégia teórico-metodológica, e sustentadas a partir do diálogo entre saúde coletiva, como campo de saber e de prática, e psicologia social da práxis, formulada por Enrique Pichon-Rivière. O eixo articulador dessa experiência é a formação em sentido amplo que se manifesta pelo princípio de indissociabilidade ensino-pesquisa-extensão, i.e., aprender-investigar-fazer, e como autoformação em um processo mútuo e de ação reflexiva, de aprender a aprender. Nesse sentido, argumenta-se que é metaformação e só pode se dar na perspectiva de diálogos de saberes e interdisciplinares.


ABSTRACT This article is based on the understanding of interdisciplinarity as a way to act against the fragmented visions present in the processes of knowledge production and socialization. The objective is to share reflections that problematize interdisciplinarity from the experience of graduate academic training in a research group whose members have diverse training and professional fields and focus on the relationships between public policy, health and human needs. The reflections were based on issues that emerged during the group's systematic meetings, and the group was understood as a theoretical and methodological strategy, and was sustained by the dialogue between collective health as a field of knowledge and practice, and the social psychology of praxis, formulated by Enrique Pichon-Rivière. The articulating axis of this experience is training in its broadest sense, manifested on the principle of the inseparability of (i.e., learning-research-doing) and as self-training in a mutual process of reflective teaching-research-outreach action, of learning how to learn. In this sense, it is argued that it is a metatraining that can only take place from the perspective of knowledge dialogues and interdisciplinarity.

Objectives: Pandemic disruptions to interviews and away rotations led applicants to rely on alternative sources of information about residency programs. We sought to compare program characteristics important to emergency medicine (EM)-bound applicants from before and after the pandemic. We also investigated the sources of information pandemic impacted applicants used during residency recruitment. Methods: This was a multi-institutional, cross-sectional survey of applicants to EM residency programs. We asked applicants about important factors in choosing a program and compared their response to results from 2019 National Residency Match Program. We also asked about alternative information sources used during this time of restricted access to programs of interest. Results: We surveyed 414 applicants from 40 medical schools and attained a response rate of 38.4%. Compared to 2019 applicants, our respondents identified morale of residents and quality of faculty as important factors in choosing a residency. Our subjects cited websites of the program and hospital affiliate, current residents, faculty/mentor advice, Reddit, and Doximity as sources of program information. Conclusions: Compared to 2019, our EM-bound applicants who, because of the pandemic, were unable to visit programs of interest valued resident morale and faculty quality as factors in choosing a residency program. Without in-person visits, our subjects also had to use both virtual resources (e.g., websites) and traditional sources (e.g., mentor advice) to investigate a program's culture, reputation, and diversity and inclusion. Residency programs should monitor their online presence now that this has become an alternative source of information for applicants during curtailment of in-person visits.

This study investigated the topic of the academic integrity among medical students and postgraduate trainees in the teaching hospitals of South Punjab, Pakistan. A cross-sectional survey was conducted involving 318 medical students and postgraduate trainees of teaching hospitals. The results found that medical students of pre-clinical years engaged in unethical behaviour, that is, exam cheating and plagiarism to cope with internal and external evaluation and the range of subjects needed to be studied. For postgraduate trainees, results showed trainees unintentionally engaged in the practice of plagiarism due to lack of understanding about what constitutes plagiarism, coupled with externally perceived pressures associated with expectations of research publication, promotions and tenured positions. To address these concerns, it is recommended that information literacy sessions for undergraduate and postgraduate medical students on plagiarism prevention and ethical practice be designed and facilitated by medical librarians in collaboration of faculty members.

BACKGROUND: Physicians must increasingly lead change for improvement in the value of health care for individuals and populations. Leadership, stewardship, and population health competencies are not explicitly part of the Accreditation Council for Graduate Medical Education (ACGME) requirements and are best appreciated in the context of Health Systems Science (HSS). HSS education is best approached at the institutional level, yet almost all graduate medical education (GME) curriculum is at the program level. We describe the process of designing and implementing an institutional HSS GME curriculum in a hospital-based sponsoring institution. METHODS: A group of diverse stakeholders drafted a curriculum to build competencies in leadership, stewardship, and population health, which was further refined by our Graduate Medical Education Committee (GMEC) and Resident Forum in the academic years 2015-2017. The refined curriculum was implemented at the institutional level of a large urban teaching hospital with over 80 ACGME accredited programs in the 2017-2018 academic year, participation was tracked and impact surveys were conducted. RESULTS: All programs participate in at least parts of the curriculum with sustained use. Annual surveys show a progression in assessment of our target competencies and/or opportunities to reflect and provide feedback. The annual program review meeting and GMEC meetings are used to troubleshoot and identify new curricular opportunities. CONCLUSION: This innovative institutional curriculum has been sustained for over four years and we believe that other training institutions with similar goals will find our experience implementing an institutional curriculum translatable to their clinical learning environment.

BACKGROUND: Medical professionalism reflects the commitment of physicians to their patients, society, themselves, and the profession. The study examined residents' attitudes towards professionalism and how these attitudes vary among the different demographic groups, namely gender, specialty, and year of residency. METHODS: A proportionate random sampling strategy was used to select the study sample. Medical residents from six specialties at a large tertiary care teaching facility were invited to participate in an online survey. The survey used the modified Learners Attitude of Medical Professionalism Scale (LAMPS), which consists of five domains: respect, excellence, altruism, duty/accountability, and integrity. Chi-square, Student t-test, one-way ANOVA, factorial ANOVA, and post hoc analysis were used to examine the attitudinal differences towards professionalism among the different demographic factors. RESULTS: The overall response rate was 82.7%. Overall, the residents' self-reported attitudes towards professionalism was positive. The highest score was for the "respect" domain (4.61), and the lowest was for "altruism" (3.67). No significant association was found between the mean scores and the three studied variables, namely, gender, specialty (surgical/nonsurgical), and level (senior/junior). CONCLUSIONS: No significant differences were observed in the overall attitude towards professionalism among the residents regarding their year of residency, gender, and specialty. The low altruism score and absence of improvement of the total score regarding the residents' increasing experience in the profession are concerns that need remedial action. Therefore, we suggest that future research look for possible explanations by using multi-institutional surveys that explore not only the residents' attitudes, but also the trainers' attitudes and practice, work situations, the hidden curriculum, and culture. Key messagesAttitudes towards professionalism among different demographic groups of residents do not show similar variations as has been reported in the literature, albeit in different sociocultural contexts.The low altruism score and absence of improvement of the total score as the residents gained more experience in the profession are concerns that need remedial action.A longitudinal study involving more than one institution for both residents and their faculty members to compare faculty scores with those of residents, while controlling for specialty and gender, may help elucidate the factors affecting attitudes towards professionalism and suggest possible means of addressing unfavourable attitudes.

RESUMEN La formación de especialistas constituye modalidad académica de posgrado. Es importante considerar los derechos que corresponden a los residentes en su etapa de formación, porque habitualmente se insiste y se reclama más el cumplimiento de los deberes. Al analizar y evaluar los aspectos metodológicos comprendidos en el Reglamento de la Residencia quedan bien definidas las obligaciones que demanda el proceso docente y a su vez los derechos que posee el personal en formación. En este trabajo se declaran las deficiencias presentes para desarrollar un proceso docente de calidad, fundamentalmente en el nivel primario de atención que cuenta con menor número de profesores con categoría docente superior y con menor número de especialistas de segundo grado y doctores en ciencia, lo que repercute negativamente en la conducción y asesoramiento del proceso docente de posgrado. Se llama a incrementar las potencialidades del claustro ante la necesidad de formar un capital humano con la calidad requerida.


ABSTRACT The specialists training constitutes the academic modality of the postgraduate course. It is important to consider the rights that correspond to residents in their training stage, because compliance with duties is usually insisted on and demanded more. When analyzing and evaluating the methodological aspects included in the Residency Regulations, the obligations demanded by the teaching process and, in turn, the rights held by the staff in training are well defined. In this work, the current deficiencies to develop a quality teaching process are declared, essentially in the primary level of attention that has a smaller number of teachers with higher teaching category and with a smaller number of second-grade specialists and doctors in science, which has a negative impact on the management and advice of the postgraduate teaching process. Finally, it is encourage increasing the potential of the staff to train human capital with the required quality.
